ABSTRACT

Objective:To explore the method and curative effect evaluation of repairing and rebuilding the long segmental bone defect after resection of long bone malignant tumors in adolescents.Methods:Reviewed our experience in 10 patients. Among them, 4 were male and 6 were female. The age was 10.2 years old (range, 6-16 years old). Including 7 cases of osteosarcoma, 3 cases of Ewing's sarcoma; 2 cases of humerus tumors, 7 cases of femoral tumors, and 1 case of tibial tumors-who presented between April 2015 and October 2019, in which the proportion of bone defect account for 71.4%-89.3% of the total length of diseased bone after tumor resection, with an average value of 80.9%. Considering the epiphyseal growth of young people, on the premise of ensuring enlarged tumor resection, we use traditional fibula autograft and prosthesis combined with steel plate, 3D printing design to retain the super-prosthesis of the epiphysis, and 3D printing bone trabecular was processed for the sake of making it grow successfully into the binding part of prosthesis and bone as we expect, simultaneously, the design of the short-force arm retaining the bilateral epiphysis can assit the steel plate and interlocking screws to enhance the stability.Results:The operation was completed successfully, and there were no complications such as nerve and blood vessel injury during the operation. After follow-up of 4-8 months, with an average follow-up of 22.9 months. One patient's review of X-ray film after 2 years of the operation showed that the transplanted fibula had a sign of bone resorption, and the internal fixation was in it's original position without obvious abnormalities. Two cases' auxiliary steel plates had fractured within one and a half to two years after surgery. With replacing the fractured steel plates, the internal fixation was good until now. According to the evaluation standards of the International Society of Limb Salvation: 2 cases of humerus osteosarcoma were respectively scored 23 points, 26 points, an average of 24.5 points; 4 cases of femoral osteosarcoma and 3 cases of femoral Ewing's sarcoma respectively scored 26 points and 25 points, 19 points, 30 points, 27 points, 24 points, 28 points (average, 25.6 points); 1 case of tibial osteosarcoma scored 26 points; all obtained relative satisfying treatment results.Conclusion:The combination of 3D printing and traditional machining techniques to produce an individualized long segmental prosthesis that preserves the epiphysis which applicated in the limb salvage treatment of bone malignant tumors can achieve excellent therapeutic effect.

ABSTRACT

Objective:To design a new automatic cap opening device consists of holding mechanism, open mechanism, improve mechanism, rotating mechanism and recycling mechanism in order to resolve the poor adaptability, complex structure and lower liability problem for specimen container in the biological specimen pretreatment system.Methods: This paper designed a automatic equipment to remove the rubber cap and screw cap. This equipment is compatible with the different specification specimen containers and the container cap, and the specimen container cap was stepped up and rotated with same power component.Results: The application of equipment has reduced the manufacturing cost and maintenance cost for specimen container, improved the system reliability, solved the current technical problems of the equipment, such as poor adaptability and lower liability. Conclusion: The design of equipment mainly adapts to CS-6400 series of automatic biochemical analyzer, and it can improve the detection efficiency of biological specimen, reduce the cross contamination and satisfy the practice necessity for clinical detection.

ABSTRACT

Objective To understand the distribution and antimicrobial resistance of pathogens isolated from patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)in Ordos area,so as to provide guidance for rational antimicrobial use.Methods Sputum culture and antimicrobial susceptibility testing results of 372 patients with AECOPD in Ordos area in 2013-2015 were analyzed,and the quality of life before acute exacerbation was assessed.Results A total of 296 strains of pathogens were isolated from 372 patients,252(85.14%)of which were gram-negative bacteria,the major were Pseudomonas aeruginosa(n=58,19.59%),Klebsiella pneumoniae(n=51,17.23%),Acinetobacter baumannii(n=50,16.89%),Stenotrophomonas maltophilia(n=22,7.43%),Escherichia coli(n=19,6.42%),and Enterobacter cloacae(n=16,5.41%);27(9.12%)were fungi,the major was Candida albicans(n=19,6.42%);17(5.74%)were gram-positive bacteria,the predominant species was Staphylococcus aureus.Patients with CAT(COPD assement test)score≥10 had higher proportion of isolating Pseudomonas aeruginosa and Acinetobacter baumannii than those with CAT score<10.Conclusion The main pathogens from patients with AECOPD are gram-negative strains,CAT score prior to exacerbation may be related to the emergence of pathogens at AECOPD.

ABSTRACT

<p><b>OBJECTIVE</b>To identify the feasibility and safety of fossa infratemporalis approach for blind-needle at sphenopalatine ganglion so as to provide anatomical evidence for the operation and the prevention of non-immediate adverse reaction.</p><p><b>METHODS</b>The variations of pterygopalatine fossae in sixty dry skulls were observed by selecting measuring points for facial skull width. The brains of six wet skulls were taken out,then acupuncture of fossa infratemporalis approach was applied. Sphenopalatine ganglion was separated accurately with the pterygopalatine segment of maxillary arteria retained in the pterygopalatine fossa after its paries posterior was opened. We detected whether the needle was inserted into pterygopalatine fossa. Measurements showed needle inserted depth, facial skull width,the distance between the needle and sphenopalatine ganglion,the distance between the needle and the pterygopalatine segment of maxillary arteria,the distance between the pterygopalatine segment of maxillary arteria and the crotaphitic nerve in pterygopalatine fossa.</p><p><b>RESULTS</b>The distance between the slight hollow under bilateral arcus superciliaris was selected as skull width, and 3 dry skulls showed the variation of pterygopalatine fossa. Needles were inserted into the pterygopalatine fossae of the wet skulls (12 times). The proportion of the inserting depth to the distance between the slight hollow under bilateral arcus superciliaris was 44%-54%. Only twice did the needle contact sphenopalatine ganglion. The average distances between the sphenopalatine ganglion and the needle were (5.88±3.70) mm in the left side and (6.43±5.54) mm in the right side. The average distances between the needle and the pterygopalatine segment of maxillary arteria were (2.77±3.99) mm left and (2.53±3.10) mm right. The average distances between the pterygopalatine segment of maxillary arteria and the crotaphitic nerve in pterygopalatine fossa were (2.83±4.05) mm left and (2.67±4.95) mm right. The mean data between the two sides had no statistic significance about all the above indices (all>0.05).</p><p><b>CONCLUSIONS</b>Fossa infratemporalis approach is feasible for blind-needle at sphenopalatine ganglion with less possibility to contact it. The effect of treating nasitis may achieved by little distance to nerve. Pricking at the pterygopalatine segment of maxillary arteria may induce non-immediate adverse reaction. The safety and efficacy should be comprehensively considered. There is a proportional relationship between the width of the skull and the insertion depth of the needle. The inserting depth of 44 percent may appropriate accounted for skull width.</p>

ABSTRACT

Objective To investigate the distribution and antimicrobial resistance in Enterococcus species isolated from Ordos Central Hospital.Methods The Enterococcus strains were isolated from clinical specimens from January 2010 to June 2013.The identification and antimicrobial susceptibility testing were completed on VITEK 2 Compact.WHONET 5.6 software was used to analyze the data.Results A total of 271 strains of Enterococcus were collected,including E.faecium (50.6%,137/271), E.faecalis (29.5%,80/271),and other Enterococcus (19.9%,54/271).The Enterococcus isolates were mainly from urine (25.5%,69/271 ),pus (14.8%,40/271 )and wound secretion (12.5%,34/271 ).The E.faecalis strains were highly susceptible to vancomycin and linezolid.Only 1 .3% and 1 .5% of the strains were resistant to vancomycin and linezolid, respectively.No strains of E.faecalis were resistant to nitrofurantoin.The percentage of E.faecalis resistant to penicillin and ampicillin was 11.8% and 2.6%,respectively.About 31.0% and 22.9% of E.faecalis strains were resistant to gentamicin (high level)and streptomycin (high level),respectively.The E.faecium strains were more resistant to most antibiotics tested than E.faecalis.The drug-resistance rate of E.faecium strains to vancomycin was 4.4%.But no strains were found resistant to linezolid.Only 19.1% of these strains were resistant to nitrofurantoin.Also 44.8% and 26.4% of E. faecium isolates were resistant to gentamicin (high level)and streptomycin (high level),respectively.However,E.faecium was less resistant to tetracycline and quinupristin-dalfopristin than E.faecalis.The resistance rate was 58.3% and 0, respectively.Conclusions The E.faecium strains are more resistant to most drugs tested than E.faecalis.Some strains are resistant to vancomycin.The resistance of Enterococcus varies widely with region and species.Antimicrobial therapy for such enterococcal infections should be based on the results of antimicrobial susceptibility testing.

ABSTRACT

ObjectiveTo investigate the protective effects of panax quinquefolium 20s-protopanaxtriol saponins (PQTS) on ventricular remodeling in rats with pressure overloaded hypertrophic myocardium.Methods Wister rats were randomly divided into operation,model,positive captopril,and low,moderate,high PQTS groups.The model of pressure overload-induced ventricular remodeling was established through the method of rat's abdominal aorta deligation.After 6 weeks of PQTS treatment ( 12.5,25.0 and 50.0 mg · kg-1 · d-1,i.p),myocardial morphological and hemodynamic parameters were determined.The content of malondialdehyde (MDA) and activity of superoxide dismutase (SOD)in serum,and the concentrations of prostacycline (PGI2),thromboxane A2 (TXA2),endothelium (ET) and angiotensin Ⅱ( Ang Ⅱ ) in plasma were also determined.ResultsCompared with remodeling group,PQTS could inhibit myocardial pathological changes,decrease significantly ventricular weight and cardiac coefficient,increase significantly systolic blood pressure,diastolic blood pressure,mean arterial pressure,left ventricular systolic pressure and the maximum left ventricular pressure rising and dropping rates(dp/dtmax),reduce the heart rate and left ventricular end-diastolic pressure in ventricular remodeling rats.PQTS could also decrease the content of MDA and enhance significantly activity of SOD in serum.In addition,PQTS could decline the contents of ET,Ang Ⅱ and TXA2 in plasma while increase significantly the content of PGI2 in plasma and PGI2/TXA2 ratio(P<0.05 or P<0.01).ConclusionsPQTS has protective effects on ventricular remodeling through improving systolic and diastolic function in ventricular remodeling rats,increasing anti-oxidase activity,reducing the damage of free radicals and vasoactive substance onmyocardium,and correcting disequilibrium of PGI2/TXA2 in ventricular remodeling rats.

ABSTRACT

Children avascular necrosis of femoral head is a self-healing,self-limiting and non-systematic disease occouring locally in femoral head of children.The purpose of traditional conservative and surgical treatments is to improve blood circulation,decrease intraosseous pressue,decrease arthrosis pressure,reduce capital mechanical pressure to the femoral head and increse its containment.Anyway,no treatment could stop the progress of coxa plana and degeneration of the bone,which could lead to disorder of the hip.As a new technique,autologous bone marrow stem cellular transplantation has obtained preliminary curative effect.It has been proved to be a new effective treatment of children avscular necrosis of the femoral head with combined use of tissue enginering bone.

ABSTRACT

[Objective]To observe the effect of cross-finger-like micro-weave suture plus sodium hyaluronate technology in the treatment of extensor tendon injury by the animal experiments.[Method]Twenty-eight Leghorn chickens were randomly divided into four groups,double-cross stitching was used in group A in the treatment of extensor tendon injury,double-cross stitching plus sodium hyaluronate in group B,cross-finger-like micro-braided suture in group C,cross-finger-like micro-braided suture plug sodium hyaluronate surgery in group D.Six weeks after surgery,morphological observation,histological observation and biomechanical determination were done by using the naked eye and microscope,as well as the results of the experiments were statistically analyzed.[Result]In the group of the cross-finger-like micro-weave suture plus sodium hyaluronate,tendon grew in good shape without nodules after surgery,pseudome was formed around tendon,tendon cells mostly arranged regularly in bunches,maximum load was (70.9 ? 5.68) N on average,it was significantly larger than those in other groups and there was a significant difference by the analysis of variance (P

ABSTRACT

The extensor tendon rupture is a common disease.Traditional management includes "8" suture and cross suture.But second operation is commonly needed for the reason of postoperative Nodules or adhesion.So a new technique is invented by the author:micro-weave suture combined with sodium hyaluronate.The new technique has improved dynamic intensity of the tendon and avoided postoperative adhesion.In this paper,this technique is reviewed.

ABSTRACT

Objective: To investigate the effect of cognitive intervention on the therapy to Chronic prostatitis(CP) following Sexually Transmitted Diseases(STD). Methods: 84 patients were divided into two groups, as control group(n=42) in which prostant was locally used and the intervention group(n=42) which received cognitive Intervention therapy based on the locally using prostant. After a month, the therapy effect was evaluated. Results: Compared with control group, the subjective syndrome in intervention group was improved much more apparent receiving therapy one month later(P 0.05). Conclusion: Cognitive intervention is an effective method to improve the subjective syndrome of CP following STD; so a clinical medicine should study psychology.

ABSTRACT

Objective To investigate the effects of anti-adhesion treatment and high-strength suture technique on the treatment of extensor tendon rupture by animal experiments and clinical application.Methods Twenty-eight leg-born chickens were randomly divided into four groups(7 each).Double cross suture was applied in group A,while double cross suture combined with sodium hyaluronate spraying in group B,cross-finger-like micro-braided suture in group C,and cross-finger-like micro-braided suture combined with sodium hyaluronate spraying in group D.The animals were sacrificed 6 weeks after operation,morphological,histological and biomechanics were observed and compared among the groups.One hundred and sixteen patients were treated with the surgical method in group D(89 males,27 females,aged 20-55 with an average of 36 years;73 with extensor tendon rupture,38 with strain/chalasis,5 with firearm injuries;56 on back of hand,48 on central slip,12 on lateral fixing chorda;82 with one-stage operation,and 34 with second-stage operation),and then followed-up for 2-5 years to observe the therapeutic effects.Results The repaired tendons in group D was in good contour,most tendon cells arranged regularly in bunches.The maximum load was significantly higher in group D(70.9?5.7N) than in group A(48.4?5.7N),Group B(51.3?3.2N) and Group C(68.3?2.8N,P
